You are invited to participate in a short survey as part of a study led by Master of Public Health candidates at the University of Toronto in collaboration with Ontario Dietitians in Public Health (ODPH). The study aims to better understand infant formula accessibility, procurement, and affordability across Ontario from the perspective of community service providers.

The survey includes up to 15 questions and will take approximately 5-10 minutes to complete. Your participation is completely anonymous and voluntary, and you may skip any questions or end the survey at any time before submission. No identifying information will appear in any reports or publications, and any direct quotes used will be anonymized.

Your contributions are greatly appreciated and will help inform a situational assessment to identify current challenges and gaps in support for families accessing infant formula.
